Pay As You Go
Payment of your estimated portion is collected at the time of service. Fees for procedures requiring multiple 
visits can be split between visits.

Care Credit
We offer no-interest payment plan options through Care Credit. Extended payment plans are also available. 
We accept all of the following forms of payment: Cash, Personal check, MasterCard, Visa, and Care Credit.

 

Insurance
Dental insurance is a contract between your employer and a dental insurance company. The benefits that 
you will receive are based on the terms of the contract negotiated by your employer and the insurance 
company. The reimbursement from your dental insurance company is based on a formula that determines 
what benefits you will receive and the percentage of the dentist  office fee that will be paid.
Our office will do everything possible to help you understand and make the most of your dental insurance 
benefits. We will complete and submit forms to your dental insurance company to achieve the maximum 
reimbursement to which you are entitled. We collect only for the portion remaining after 
estimated insurance benefits have been applied. The financial obligation for dental treatment is between 
you and our office. The insurance company is responsible to you and not to our office. Once your carrier has 
paid the claim, any difference will be due upon receipt of our statement.

Missed Appointment Fee
If for any reason you cannot keep your scheduled appointment, please notify us as soon as possible. There 
will be a $50 missed appointment fee if we have not received notice from you at least 24 hours in advance 
of your scheduled appointment time.
